1. You must be a permanent resident of Cooke County.
2. A driver’s license or official picture id.
3. An official document with name and physical address of applicant.
4. A second contact person who serves as a reference. They must also be a Cooke County resident
and have a different address and phone number than the applicant.
5. If getting the card for a child, then parents or the person with custody of the minor may fill out
the registration.
6. The borrower must pick up the card at the library.
7. Applications for library cards are not accepted a half-hour (1/2) before closing. This means that
on weekdays after 5:30 pm, except on Tuesdays after 6:30 pm and on Saturdays after 2:30 pm
a patron CAN NOT register for a card.
8. If a replacement card is needed the cost is $3.00 for the first lost card, $4.00 for the second one
and $5.00 for the third or more.
